TERMS AND CONDITIONS FOR ALUMNI AND STUDENT PARTICIPANTS IN WAKE FOREST UNIVERSITY SCHOOL OF MEDICINE ALUMNI NETWORK PROGRAMS

The Wake Forest University School of Medicine Alumni Network programs are intended for use only by alumni and students of the Wake Forest University School of Medicine. These programs are intended to help students and alumni connect with one another. These programs are to be used by participants solely for such purposes. Program participants must read and agree to comply with these Terms and Conditions in order to be eligible to participate in Wake Forest University School of Medicine Alumni Network programs. A participant’s failure to comply with these Terms and Conditions may result in removing the participant’s access to the Wake Forest University School of Medicine Alumni Network Online Portal and/or dismissal from Wake Forest University School of Medicine Alumni Network programs. Student participants may also be subject to disciplinary action by the School of Medicine in accordance with the School’s policies.

Use of Participant Information: I understand that all constituent information in the Wake Forest University School of Medicine Alumni Network is to be used only for the legitimate purposes related to the Wake Forest University School of Medicine Alumni Network programs (hereinafter “Programs”). I will not share this information with others or use it for any purposes unrelated to the Programs.

Confidentiality: I understand that conversations between Wake Forest University School of Medicine Alumni Network program participants should be treated as confidential, unless otherwise mutually agreed to by the parties or unless disclosure is necessary to report a violation of these Terms and Conditions to the School of Medicine.

Appropriate Conduct: I understand that participants in the Programs shall not engage in conduct that is inconsistent with the goals, objectives, and policies of the Programs, the Medical Alumni Association (MAA), or Wake Forest University School of Medicine. Participants are required to comply with the School of Medicine’s non-discrimination policy which prohibits discrimination, including harassment, on the basis of race, color, religion, national origin, sex, age, sexual orientation, gender identity and expression, genetic information, disability, and veteran status.

Meeting Commitment: I understand that participants are expected to promptly respond to host/host student/mentor/mentee and carry out communications commitments that are made to meet with students or alumni or, if necessary, reschedule. In-person mentoring meetings (which are not required) must be held in public spaces – conference rooms, coffee shops, classrooms, etc. - to protect the comfort and safety of all participants. You understand and agree that you are solely responsible for taking all necessary precautions when meeting individuals through the Programs.

Your Profile: I understand that participants must promptly complete and update their online profile, including the times when the participant is available to meet with other participants and the types of meetings the participant is willing to undertake. I also understand that each participant is solely responsible for, and assumes all liability regarding, the information and content contributed to the profile and the Programs.

Appropriate Use of the Online Portal: The Wake Forest University School of Medicine Network Online Portal contains personal alumni and student profiles and messaging functions. You agree to use these messaging functions only to post, send and receive messages containing material that is appropriate for and in connect with use in the Programs. By way of examples only, you agree that when using the Wake Forest University School of Medicine Network Online Portal you will not do certain things, including but not limited to the following: A. Use the messaging functions to distribute or promote spam or any duplicative or unsolicited messages. B. Harass, threaten or otherwise violate the legal rights of others. C. Post or distribute any inappropriate or obscene topic, material or information. D. Harvest or otherwise collect information about others, including phone numbers and email addresses, for purposes unrelated to the Programs. E. Create a false identity or profile for the purpose of misleading others. F. Interfere with or disrupt the service or networks connected to the service.

Transportation Safety: If you voluntarily agree to drive your host/host student/mentor/mentee in a car, you understand that you assume personal responsibility for the safety of yourself and your passengers. You understand that Wake Forest School of Medicine has no liability for use of personal vehicles and any damage or injury that may result from use of such vehicles.

Release, Waiver, Discharge and Covenant Not to Sue: I agree that I am participating in this program voluntarily, and I agree that I will not hold Wake Forest University School of Medicine, Wake Forest University Health Sciences, Wake Forest University, and their respective employees, officers, trustees and agents, (collectively, “the Releasees”) responsible or liable for any injury, harm, damage, or loss, regardless of cause, that I may sustain or incur in connection with my participation in Wake Forest University School of Medicine Alumni Network programs. To the maximum extent permitted by applicable law, I hereby release, waive, promise not to sue, and discharge the Releasees from any and all liability, claims, demands, actions and causes of action whatsoever for any and all injury or damages, whether direct, indirect, general, special, compensatory, consequential, and/or incidental, to my person or property arising out of or related to my participation in the Wake Forest University School of Medicine Network. I further hereby agree to defend, indemnify and save and hold harmless the Releasees, collectively and individually, from any loss, liability, damage or costs that they may incur relating to my participation in the Wake Forest University School of Medicine Network programs.

I hereby further represent that, should I bring another Program participant to my workplace, I will secure any and all necessary permissions from my employer for the participant to be present, and that, as between me and the Releasees, I will assume any and all responsibility for the actions of the participant and shall defend, indemnify and save and hold harmless the Releasees and each of them, from any loss, liability, damage or costs they may incur relating to the participant’s actions in my workplace.

Terms and Conditions Changes: The Wake Forest University School of Medicine, including the Office of Philanthropy and Alumni Relations, may change these Terms and Conditions for the Wake Forest University School of Medicine Alumni Network from time to time, and in the School of Medicine’s sole discretion. Participants in the Programs should frequently check this page for any changes to these Terms and Conditions. Your continued participation in the Wake Forest University School of Medicine Alumni Network after any change in the Terms and Conditions will constitute your acceptance of such change.

Resignation: If and when a participant is no longer willing or able to participate in the Programs, the participant must notify the program administrator of this fact so that the participant’s profile can be removed from the online portal. Email alumni@wakehealth.edu to be removed.

Reporting Violations: Please report any violations of these Terms and Conditions to alumni@wakehealth.edu.

I understand, acknowledge and voluntarily agree to comply with the Terms and Conditions of being a participant in the Wake Forest University School of Medicine Network programs.
